1 | SENATE RESOLUTION
| ||||||
2 | WHEREAS, The members of the Illinois Senate are saddened to | ||||||
3 | learn of the death of Dr. Phillip W. "Phil" Conn of Danville, | ||||||
4 | who passed away on July 31, 2020; and
| ||||||
5 | WHEREAS, Dr. Conn was born in Gary, Indiana to Clyde and | ||||||
6 | Hermine Conn on January 29, 1950; he graduated from the Indiana | ||||||
7 | University School of Dentistry in 1978; and
| ||||||
8 | WHEREAS, Dr. Conn Phillip started his practice in Danville | ||||||
9 | and practiced dentistry for 42 years; and
| ||||||
10 | WHEREAS, Dr. Conn was the former president of the Indiana | ||||||
11 | University School of Dentistry Alumni Association and the | ||||||
12 | former president of the Danville District Dental Society; he | ||||||
13 | was a member of the American Dental Association, the Illinois | ||||||
14 | State Dental Society, the Academy of General Dentistry, the | ||||||
15 | American Academy of Cosmetic Dentistry, and the Dean's Society | ||||||
16 | of the Indiana University School of Dentistry; he was also one | ||||||
17 | of the founding dentists of the free dental services program at | ||||||
18 | Danville's first community health clinic, and he volunteered | ||||||
19 | his time and services, offering free back to school student | ||||||
20 | dental check ups for many years; and

21 | WHEREAS, Dr. Conn received numerous honors from |

| |||||||
1 | professional and service organizations, including the | ||||||
2 | Distinguished Service Award from the Indiana University School | ||||||
3 | of Dentistry; and
| ||||||
4 | WHEREAS, Dr. Conn was an active member of the Second Church | ||||||
5 | of Christ in Danville; he took part in two missionary trips to | ||||||
6 | the south coast of Africa with his church, where he performed | ||||||
7 | dental work in the clinic; he enjoyed spending quality time | ||||||
8 | with his family, meeting with friends, walking his dogs, | ||||||
9 | traveling, golfing, fishing, hunting, playing the guitar, | ||||||
10 | woodworking, gardening, boating, and dancing with his wife; and
| ||||||
11 | WHEREAS, Dr. Conn was preceded in death by his parents; and
| ||||||
12 | WHEREAS, Dr. Conn is survived by his wife of 18 years, | ||||||
13 | Kandis Conn; his daughters, Stacy (Stephen) Ellis, Leanne | ||||||
14 | (William) Moore, and Holly (Thomas) Slaters; his stepdaughter, | ||||||
15 | Jennifer (Marek) Dziedzic; his brother, Joseph Conn; his niece, | ||||||
16 | Rachel Conn; and seven grandchildren; therefore, be it

17 | RESOLVED, BY THE SENATE OF THE ONE HUNDRED FIRST GENERAL | ||||||
18 | ASSEMBLY OF THE STATE OF ILLINOIS, that we mourn the passing of | ||||||
19 | Dr. Phillip W. "Phil" Conn and extend our sincere condolences | ||||||
20 | to his family, friends, and all who knew and loved him; and be | ||||||
21 | it further
