Summary

Requires that operator or proprietor of tanning facility witness signing of written statement by parent or legal guardian of minor before minor is allowed to use tanning device; provides requirements for written statement; prohibits minor younger than certain age from using tanning device at tanning facility; deletes provisions authorizing use of tanning device by certain minors if accompanied by parent or legal guardian; allows use of tanning device by certain minors as treatment prescribed by health care provider.
