Summary
Requires the Insurance Commissioner to develop a uniform prescription drug prior authorization form no later than July 1, 2013, for only prescription drugs for the treatment of diabetes or any form of cancer. Beginning January 1, 2014, requires the uniform prescription drug prior authorization form to be used by prescribers and health care insurance providers. Effective July 1, 3000, and repealed on July 1, 2017. (SB2436 HD2)
