Summary

This bill would require hospitals to use a standardized form to provide families and patients the following information concerning the continual care of the patient at the time of discharge: * What to expect during the patient's normal course of recovery * What symptoms require the calling of a physician * What symptoms require going back to the hospital Often upon discharge the patient has not fully recovered; rather the patient is considered to be in good enough condition to continue his recovery outside of the hospital. Some hospitals do a good job of preparing the patient and his family for this stage of recovery but most do not. By requiring hospitals to provide this basic information, care provided to discharged patients is expected to improve at almost no additional costs.
