Bill Title: Facilitates enrollment in the child health insurance plan; directs the commissioner of education, in consultation with the commissioner of health, to develop a form with information regarding the existence of the child health insurance plan; further directs the commissioner of education to promulgate rules and regulations to require all public schools and school districts to disseminate such materials to families of students.
